Physical examination
Our board-certified immigration physician, supported by his team, performs the exam. The process involves a complete medical history review and a physical evaluation.
Photo ID
Please present one of the following documents (Minors must also bring photo ID)
  • Driver’s License
  • Passport
Vaccination/Immunization Record
  • If your vaccination record was issued outside the United States, it must be validated by the Florida Department of Health.
  • If you do not have a vaccination record, we Offer antibody testing services to verify immunity (see prizes above)
  • If you do not have the vaccines, you can get them at any pharmacy and bring the record.
Labs to be performed:
The following labs are performed at the center; fasting is not required.
Patients over 14 years of age:
  • RPR (Syphilis)
  • Neisseria gonorrhoeae (Gonorrhea)
Patients under 14 years of age:
  • TB (Tuberculosis)
USCIS Form I-693
A crucial component of the process is Form I-693, which records the results of the medical examination and vaccination history. Once completed, it is stamped and provided to the applicant for submission to U.S. Citizenship and Immigration Services (USCIS).
